If you plan to use your insurance, please

1) Bring your card and driver’s license with you for the first visit.
2) Call your insurance company before your first visit to find out
        - if your therapist is covered by your insurance (”in-network”)
        - how much you will need to pay each visit 
        - how many visits you are allowed each year (calendar or plan year)
        - if your yearly visit limit for mental health services is split between all providers
        - if you are allowed to see two or more mental health providers in one day
 3) Stay apprised of any and all changes in your insurance plan. If you are terminated, you are responsible for any charges your insurance won’t pay.


Also, make sure that you

  1. Carry your insurance plan card(s) with you at all times. You may have more than one card for your medical plan, prescription plan, dental plan, and so forth.
  2. Carry some form of photo identification with you at all times.
  3. Review your health insurance benefits before you need them. By doing so, you will know what to expect at the time of need.
  4. Review your health insurance plan limitations and exclusions before you need the coverage. Again, this will help you know what to expect at the time of need.
  5. Know your deductible amount.
  6. Know your co-pay amount.
  7. Know your stop loss amount.
  8. Check with your health insurance company to determine if it considers SEB Health to be “In-Network” or “Out-of-Network” for reimbursement purposes.
